Abstract Objective To investigate the expression of serum miR-302b in elderly patients with acute myocardial infarction (AMI) and its clinical significance. Methods A total of 138 elderly AMI patients admitted to our hospital and 65 normal controls were selected to compare the changes of serum miR-302b, myocardial troponin I (cTnI) and creatine kinase isoenzyme (CK-MB) levels between the two groups. The value of serum miR-302b, cTnI and CK-MB levels in the diagnosis of AMI in elderly patients was analyzed by ROC curve. Pearson correlation was used to analyze the correlation between the levels of miR-302b and cTnI, CK-MB in elderly patients with AMI. Results The levels of serum miR-302b (4.25±1.30 vs. 0.51±0.16), cTnI (3.17±1.36 vs. 0.04±0.01, ng/mL) and CK-MB (73.50±19.36 vs. 11.70±2.25, U/L) in the AMI group were significantly higher than those in the control group (P<0.01). ROC curve analysis showed that the critical values of miR-302b, cTnI and CK-MB in the diagnosis of elderly AMI were 2.73, 1.52 ng/mL, 61.30 U/L, respectively. The AUC [0.935, 95%CI (0.872-0.985)] of three combined diagnosis of elderly AMI was the largest, with sensitivity and specificity of 94.8% and 89.2%. Relevant analysis showed that serum levels of miR-302b were positively correlated with cTnI and CK-MB in elderly patients with AMI (r=0.826, r=0.745, P<0.01). Conclusion Serum miR-302b is highly expressed in elderly AMI patients, and the combined detection with cTnI and CK-MB is helpful to improve the accuracy of AMI diagnosis in elderly patients.
