Hyperbaric oxygen combined with alprostadil in myocardial injury of ACOP |

Abstract Objective To investigate the clinical effects of hyperbaric oxygen (HBO) combined with alprostadil on myocardial injury induced by acute carbon monoxide poisoning (ACOP). Methods Eighty-eight patients with ACOP who were treated in our hospital from October 2013 to March 2018 were randomly divided into two groups: control group (n=43) given HBO treatment and observation group given HBO+alprostadil treatment (n=45). The clinical efficacy, clinical indicators, myocardial enzymes aspartate aminotransferase (AST), lactate dehydrogenase (LDH), creatine kinase (CK), CK isoenzyme (CK-MB), and myocardium troponin T (cTnT) levels were analyzed before and after treatment in both groups. Results After 5-day treatment, the total effective rate was 91.11% in the observation group and 82.22% in the electrocardiogram were significantly higher than those in the control group (74.42% and 62.80%) (P<0.05). Before treatment, the electrocardiogram index, the myocardial enzyme spectrum and cTnT in the two groups were observed. There was no significant difference in the levels of AST, LDH, CK, CK-MB and cTnT (P>0.05). After 5-day treatment, the levels of myocardial enzymes[AST(26.11±6.54)U/L vs.(33.14±7.63)U/L], LDH(211.23±33.12)U/L vs.(260.69±38.23)U/L, CK(107.46±28.18)U/L vs.(189.37±42.45)U/L, CK-MB(11.23±4.12)U/L vs.(14.56±5.31)U/L, and cTnT(0.15±0.05)μg/L vs.(0.27±0.12)μg/L] were significantly lower in the observation group compared with the control group(P<0.05). The average recovery time[(3.12±1.37)h vs.(5.03±1.48)h], hospitalization days[(6.02±1.89)d vs. (8.12±1.76)d], hospitalization costs[(1235.65±334.12)yuan vs.(1632.88±478.35)yuan], and hyperbaric oxygen chamber treatment times[(11.78±3.28)times vs.(15.76±4.35)times] in the observation group were significantly lower than those in the control group (P< 0.05). Conclusion HBO combined with alprostadil has obvious curative effect on patients with myocardial injury caused by ACOP. It could promote early recovery of brain electrical activity, shorten awakening time and length of stay, reduce occurrence of late encephalopathy, and show good prognosis, which may be related to the reduction of myocardial enzymes and cTnT levels.
