Effects of heated humidified high flow nasal cannula oxygen therapy on respiratory failure in patients with acute stroke-associated pneumonia |

Abstract Objective To observe the effect of heated humidified high flow nasal cannula oxygen therapy (HFNC) on respiratory failure in patients with acute stroke-associated pneumonia. Methods 81 patients with respiratory failure caused by acute (within 24 hours) stroke-associated pneumonia (76 case completed) were selected in the department of neurology of Wuxi People′s Hospital. They were randomly divided into HFNC group (n=34) and conventional oxygen therapy group (n=42). The neurological impairment score (NIHSS) and clinical pulmonary infection score (CPIS) of the two groups were recorded before and after treatment. At the same time, modified Rankin score (mRS) of the two groups were also recorded in 90 days. Results There was no significant difference in the number of patients who died or changed to mechanical ventilation between HFNC group and conventional oxygen therapy group (P>0.05). HFNC improved CPIS in 7 days and 14 days and NIHSS in 14 days in acute stroke-associated pneumonia patients (P<0.05). mRS was lower in HFNC group (2.88±1.17)scores in 90 days than in the conventional oxygen therapy group [(3.60±1.53)scores, P<0.05]. Conclusion HFNC can improve lung infection in patients with stroke-associated pneumonia. At the same time, it can improve the recovery of the neurological deficit and prognosis in acute stroke.
