Abstract Objective To investigate the joint value of Kidney Disease: Improving Global Outcomes (KDIGO) criteria combined with 48 h-Δ Sequential Organ Failure Assessment (SOFA) score in predicting the prognosis for patients with acute kidney injury (AKI) caused by sepsis. Methods Clinical data of septic patients who in Department of Intensive Care Medicine, Weifang Peoples Hospital from March 2012 to March 2013 were collected. Clinical features, phase of KDIGO, SOFA score, the length of stay in ICU and the mortality in 28 days were studied. The patients were divided into two groups according to the 48 h-ΔSOFA. Each group was divided into three subgroups according to KDIGO criteria. The relationship between phase of KDIGO plus 48 h-ΔSOFA and the length of stay in ICU as well as mortality were analyzed. Results Of the 168 admissions, in 48 h-ΔSOFA > 0 group (n=98) 20 (22.4%) patients died, and in 48 h-ΔSOFA≤0 group (n=70) 30 (42.9%) patients died (P<0.05). KDIGO stage 3 had a higher odds ratio value in predicting nonsurvival (OR 2.5, 95% CI 2.1~2.9, P=0.01) and a longer stay in 48 h-ΔSOFA > 0 group (P<0.05); KDIGO stage 2 (OR 2.71, 95%CI 2.33~3.10, P=0.01) and KDIGO stage 3 (OR 4.52, 95%CI 4.21~5.13, P=0.01) had a higher odds ratio value in predicting nonsurvival in 48 h-ΔSOFA≤0 group. Conclusion The combination of KDIGO criteria and 48 h-ΔSOFA score is an effective indicator to evaluate the prognosis and to stratify patients with AKI caused by sepsis. More attention should be paid to the patients with high KDIGO staging and 48 h-ΔSOFA≤0 patients because of worse prognosis, the patients with high KDIGO staging and 48 h-ΔSOFA>0 patients because of its longer stay in ICU.
