Prognostic value of miR-27a, miR-b and PESI in the patients with acute pulmonary embolism |

Abstract Objective To investigate the prognostic value of serum miR-27a, miR-27b and pulmonary embolism severity index (PESI) in the patients with acute pulmonary embolism (APE). Methods One hundred and sixty APE patients admitted to Haikou Third People′s Hospital from January 2016 to June 2018 were divided into survival group (n=115) and death group (n=45) according to the 28-day survival of APE patients. APE patients were divided into low risk group (n=34), medium risk group (n=68) and high risk group (n=58) according to PESI score. Real-time fluorescence quantitative polymerase chain reaction (RT-PCR) was used to detect the changes of serum levels of miR-27a and miR-27b in each group. The prognostic value of serum levels of miR-27a, miR-27b and PESI in patients with APE was analyzed by ROC curve. Pearson correlation was used to analyze the correlation between serum levels of miR-27a, miR-27b and PESI in APE patients. Results The levels of serum miR-27a (4.26±0.93 vs. 1.95±0.42), miR-27b (3.72±0.75 vs. 1.63±0.51) and PESI (164.70±28.60 vs. 93.50±14.30) in the death group were significantly higher than those in the survival group (P<0.01). The levels of serum miR-27a (3.94±0.85 vs. 2.87±0.62, 1.52±0.34), miR-27b (3.60±0.71 vs. 2.26±0.54, 1.18±0.20) and PESI (158.50±26.40 vs. 102.40±18.50, 75.20±10.70) in high-risk group were significantly higher than those in medium-risk group and low-risk group(P<0.01). ROC curve analysis showed that the best cut-off values of serum levels of miR-27a, miR-27b and PESI for predicting APE mortality were 3.45, 2.96 and 135.70, respectively. Three joint forecast APE mortality of the AUC [0.862 (95%CI0.803~0.926)] is the largest, and its sensitivity and specificity were 87.2% and 82.5%, respectively. Correlation analysis showed that serum levels of miR-27a, miR-27b were positively correlated with PESI in the death group (r=0.795, r=0.718, P<0.01), miR-27a and miR-27b levels were positively correlated (r=0.746, P<0.01). Conclusion The up-regulation of the expression of miR-27a and miR-27b is related to the prognosis of APE patients. Moreover, miR-27a and miR-27b combined with PESI can effectively evaluate the prognosis of APE patients.
