Efficacy observation on alprostadil in treatment of myocardial injury and renal function by moderate and severe acute carbon monoxide poisoning |

Abstract Objective To observe the early curative effect of alprostadil on myocardial injury renal function in patients with moderate and severe acute carbon monoxide poisoning. Methods From November 2015 to January 2018, 136 patients with moderate and severe acute carbon monoxide poisoning were enrolled in our study. The patients were randomly divided into the alprostadil group and the control group. Both groups were treated with hyperbaric oxygen, oxygen inhalation, preventing cerebral edema, and promoting brain cells function recovery. The patients in the alprostadil group were treated with alprostadil 10 μg once daily for 7 days. Indexes of myocardial ischemia and injury, electrocardiogram, cardiac function, renal function and early prognosis were observed in the two groups before and after treatment. Results There were significant differences in modified albumin (71.71±10.10) U/mL, creatine kinase MB (68.84±24.56) mg/mL and troponin I (1.12±1.56) ng/mL compared with control group after 6 hours of treatment with alprostadil (P<0.05). There was no statistical difference in myoglobin (Myo) between the two groups after alprostadil treatment. The QTc (430.75±7.82) ms and heart rate (67.59±8.54) beat/min were significantly improved compared with the control group (P<0.05) 7 days after alprostadil treatment. There was no significant difference in cardiac systolic function and diastolic function between the two groups. The cystatin C (0.57±0.17) mg/L was improved significantly compared with the control group (0.68±0.20) mg/L (P<0.05) 7 days after alprostadil treatment. There was no significant difference in early prognosis between the two groups. Conclusion Treatment of alprostadil could reduce myocardial ischemia, lower myocardial injury, improve glomerular filtration rate, and protect myocardium and renal function in the early stage of acute carbon monoxide poisoning. But there is no significant effect on cardiac function and early prognosis.
