Value of serum levels of PCT, sTREM-1 and D-dimer in evaluating prognosis of sepsis patients |

Abstract Objective To explore the prognostic value of serum procalcitonin (PCT), soluble myeloid cell trigger factor-1 (sTREM-1) and D-dimmer in sepsis. Methods Ninety-six sepsis patients admitted to Haikou Maternal and Child Health Hospital were divided into survival group (62 cases) and death group (34 cases) according to the 28-day outcome. The serum PCT, sTREM-1 and D-dimmer levels of sepsis patients on the first, third and seventh days were detected respectively. Multivariate logistic regression was used to analyze the risk factors of death in sepsis patients. The value of serum levels of PCT, sTREM-1 and D-dimmer in predicting death in sepsis patients was analyzed by ROC curve. Pearson correlation was used to analyze the correlation of serum PCT, sTREM-1 and D-dimmer levels in sepsis patients. Results The serum levels of PCT (μg/L: 9.16±3.28 vs 4.75±1.20, 17.63±6.85 vs 2.94±1.08, 28.42±13.90 vs 0.68±0.20), sTREM-1 (ng/L: 138.50±27.40 vs 110.25±24.17, 150.26±30.74 vs 116.30±21.35, 161.82±32.74 vs 108.47±20.62) and D-dimmer (mg/L: 2.30±0.94 vs 1.82±0.52, 2.92±1.05 vs 2.06±0.70, 3.84±1.12 vs 1.193±0.85) in the death group were significantly higher than those in the survival group on the first, third and seventh days (P<0.05). Multivariate logistic regression analysis showed that elevated levels of serum PCT (2.417, 95% CI1.583-4.830), sTREM-1 (1.722, 95% CI1.193-3.316) and D-dimmer (1.508, 95% CI: 1.046-2.417) were independent risk factors for death in sepsis patients (P<0.05). ROC curve analysis showed that the area under the curve (0.928, 95%CI0.865-0.984) of serum PCT, sTREM-1 and D-dimmer levels combined to predict mortality in sepsis patients was the largest, with a high sensitivity and specificity of 93.0% and 88.2%. The correlation analysis showed that serum PCT were positively correlated with the levels of sTREM-1 and D-dimmer (r=0.837, P<0.01, r=0.795, P<0.01). Conclusion The levels of serum PCT, sTREM-1 and D-dimmer are correlated with the severity of sepsis, and three combined tests on the third day are of high value in predicting poor prognosis of sepsis patients.
