Modified-DECAF score in the outcome evaluation of non-invasive ventilation in AECOPD patients with do-not-intubate order |

Abstract Objective To explore the modified DECAF score in the outcome evaluation of non-invasive ventilation(NIV) in acute exacerbation of chronic obstructive pulmonary disease(AECOPD) patients with do-not-intubate order(DNI). Methods This study prospectively recruited 90 consecutive AECOPD patients requiring NIV with DNI. The modified DECAF, DECAF, APACHE Ⅱ scores were used to calculate within 24 hours of admission. Clinical end points were death in 30 days after hospital admission. The discriminatory value of the scoring systems was determined using the area under the receiver operating characteristic (AUROC) curve. Results AUC of modified DECAF[0.875, 95%CI0.805~0.945)] for prognosis was significantly greater than that of DECAF[0.784(95%CI0.681~0.887)], APACHEⅡ[0.741(95%CI0.628~0.854)]. Conclusion The modified DECAF score has predictive values in assessing outcome of AECOPD patients requiring NIV with DNI, and better than DECAF and APACHEⅡscore .
