Abstract Cardiac arrest(CA) is one of the most common severe acute illnesses. With the development of cardiopulmonary resuscitation, the rate of return of spontaneous circulation has been greatly improved, but patients often have irreversible brain damage. Therefore, cerebral resuscitation is very important. It is currently thought that the mechanism of cerebral microcirculatory disturbance after cardiac arrest is intravascular stasis, also known as "no-reflow" phenomenon, and pial arteriolar contraction. Hypothermia therapy is considered to be the only effective neuroprotective strategy for patients with out-of-hospital cardiac arrest, but its effect on cerebral microcirculatory blood flow remains controversial. In addition, there are other studies suggest that intervention of coagulation can improve cerebral microcirculatory disturbances. This article reviews the mechanisms and its interventions of cerebral microcirculatory disturbance after cardiac arrest.
