A clinical random control trial on the clearance of lactic acid in septic shock patients |

Abstract Objective To compare the effects of early goal-directed therapy (EGDT) and lactate clearance rate in septic shock patients. And to investigate whether the albumin should be used in the early stage of resuscitation.Methods A random control trial was conducted, in which patients satisfied with the criteria of septic shock diagnosis were enrolled. A total of 92 patients were randomly divided into two groups: EGDT group and Lactic acid group.Results There was no significant difference between the two groups in survival (death rate in ICU 13.3% vs. 15.6%,P=0.369; death rate in 90 d 35.6% vs. 36.2%, P=1.019), ventilation period[(93.8±32.0) h vs. (102.5±58.9) h,P=0.564)], and ICU period [(9.3±2.6) d vs. (8.8±3.1) d, P=0.656)]. While it was fewer in Lactic acid group than EGDT group [(1861±114)mL vs. (1503±201) mL,P=0.098)], and better APACHE Ⅱscore (EGDT group P=0.008, Lactic acid group P=0.034). In the early stage of resuscitation, albumin was beneficial for reducing fluid volume and time consuming.Conclusion Lactate clearance rate is a valid index for resuscitation in septic shock patients. Especially in the hospital without central venous catheterization, it should be take into account.
