Association of heat shock protein 70 gene polymorphism and disease severity and prognosis in septic patients |

Abstract Objective To investigate the genetic association between heat shock protein 70(HSP70) gene polymorphism and disease severity and prognosis in septic patients. Methods A total of 182 septic patients from January 2014 to January 2017 were recruited in this study and were divided into the sepsis group (40 cases) and the septic shock group (142 cases). Then the patients were divided into survival and death subgroups according to their outcomes on 28th day after onset. The genotype was determined by polymerase chain reaction direct sequencing for the polymorphism of the HSP70-1 gene+190G/C, HSP70-2 gene+1267A/G and HSP70-hom gene+2437T/C. Results There was significant difference in allele distribution of HSP70-2 gene+1267A/G polymorphism between sepsis group and septic shock group (P<0.05). However, genotype and allele frequencies of HSP70-1 gene+190G/C and HSP70-hom gene+2437T/C did not differ significantly between groups (P all>0.05). Acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score and sequential organ failure analysis (SOFA) score of patients with AA genotype in HSP70-2 gene+1267A/G both were higher than those in genotype AG and GG carriers (Pall<0.05). There was no significant difference in APACHE Ⅱ score and SOFA score among different genotypes of HSP70-1 gene+190G/C and HSP70-hom gene+2437T/C (P all>0.05). In the sepsis group, there was no significant association between HSP gene polymorphism and mortality (P all>0.05). In the septic shock group, there was significant difference in allele distribution of HSP70-2 gene+1267A/G polymorphism between survival subgroup and death subgroup (P<0.05). While, genotype and allele frequencies of HSP70-1 gene+190G/C and HSP70-hom gene+2437T/C did not differ significantly between groups (Pall>0.05). Conclusion Polymorphism of the HSP70 gene may associate with disease severity and prognosis of septic patients.
