Relations between coagulation-fibrinolytic system and pulmonary artery obstruction index in pulmonary embolism |

Abstract Objective To investigate the relationship between the platelet(PLT), fibrinogen (FIB), d-dimer(D-D)and fibrinogen degradation products(FDP)levels and pulmonary artery obstruc⁃ tion index(PAOI)in pulmonary embolism(PE)patients pre- and post-treatment. Methods We con⁃ ducted a prospective study on PE patients diagnosed by CTPA in 4 hospitals in Guangxi province from 1 Jan 2015 to 31 Mar 2016. The PAOI was assessed according to the Mastora scoring method. The PLT, FIB, D-D, FDP levels and CTPA were examined pre-treatment and on the 14th-day post-treatment. The relationships between these parameters and PAOI were also analyzed pre- treatment and on the14th-day post-treatment. Results A total of 42 patients with PE were enrolled from the four hospitals. The PAOI were negatively correlated with PLT levels(r=-0.474, P=0.020), positively correlated with DD and FDP levels(r= 0.535, 0.392, respectively; P=0.000, 0.010, respectively), not correlated with FIB levels pre-treatment. The PAOI were positively correlated with D-D and FDP levels(r= 0.534, 0.481, respectively; P=0.000, 0.001, respectively), not correlated with PLT and FIB levels on the 14th- day post- treatment. The PAOI variations were correlated with PLT, D- D and FDP variations(r= 0.399, 0.556 and 0.460, respectively; P=0.009, 0.020 and 0.002, respectively), not correlated with FIB varia⁃ tions pre- and 14th-day post-treatment. Conclusion The PLT, D-D and FDP levels have correlation with PAOI, especially the relations between D-D levels and PAOI are closer; they all have a certain clini⁃ cal significance in predicting the degree of pulmonary embolus dissolution.
